VSCode lang server should have a UI that tells the user when an extension crashed. #70844
Description
See existing VS code for IExtensionManager.
The UI for VSCode should be similar. Likely a toast in the lower right that tells the user the issue and gives them Enable, Enable & Ignore, and Show stack trace.
